IKDD is an electronic database that explores mobile technology and partnership with patient to ensure completeness of source documentation collection. A mobile application with interfaces for healthcare provider and patient has been created. After a simple registration at the time of first contact with healthcare services, both can upload source documentation as pictures (of physical records, prescription slips, investigation reports etc.) or text. The data is indexed to the unique hospital ID of the patient that is captured at the time of registration. Once registered, the patient can upload his or her physical records, prescriptions and investigations as would be done from time to time. At the backend, the mobile application is linked to a database that has modules to facilitate creation of structured forms for disease registries and/or bio-banking.
