Methods: PubMed, Embase and WoS were screened for manuscripts including transplanted patients outside MC. Subgroups were made within or outside UCSF/UT7 criteria. Pooled and individual participant data (IPD) meta-analyses were performed. A comparative analysis was also made with OS outcomes of patients transplanted for standard+ HCC indications (n= 19,516) and controversial but accepted (n= 30,599) indications from the UNOS database (PROSPERO CRD4202022268510).

Results: A total of 5915 patients transplanted outside MC were included. Among them, 4963 (3315 outside and 1648 inside) according to UT7 criteria and 952 (858 outside and 94 inside) according to UCSF criteria. The 5-year OS rates of inside and outside UT7 criteria were 67.8%[64.8-70.6%] and 54.5%[52.3-56.6%], respectively, and 75.1%[68.2-83.9%] and 39.7%[35.9-43.4%] inside or outside UCSF criteria, respectively.
