Purpose
Undiagnosed chronic antibody mediated rejection (cAMR) is a concern for heart transplantation (HT) recipients who have elevated donor-specific antibody (DSA) levels and/or graft dysfunction but a negative endomyocardial biopsy (EMB). Donor-derived cell-free DNA (dd-cfDNA) and cardiac magnetic resonance (CMR) imaging may be used to provide diagnostic clarity in these cases. Methods
HT recipients from 2000-2022 with DSA and/or graft dysfunction (TTE; LVEF<40%) without evidence of rejection by EMB who underwent CMR were included.
