Discussion: EBV is commonly characterized by fever, malaise, sore throat, upper respiratory symptoms, headache, and lymphadenopathy. CNS involvement like encephalitis, meningitis, or seizure is uncommon. When aseptic meningitis is suspected in a young immunocompetent patient, other more common viruses should be excluded first. Unlike many other viral infections, acute EBV infection can also cause rhabdomyolysis leading to AKI. Early detection and treatment of the complication with aggressive fluid is essential to prevent further renal damage. Our patientLs symptoms, including mentation, improved within a week with symptomatic and supportive treatment.
Conclusion: Atypical presentations of EBV infection like CNS involvement and rhabdomyolysis are possible in young adults, especially in the absence of other common CNS viral infections.
