Pardeshi and coauthors published an interesting study on the effect of glycated hemoglobin trajectories on outcomes in patients with pulmonary tuberculosis in India in the February 2024 issue of Chest. 1 This study included 588 patients who were classified by HbA1c trajectories based on levels checked at baseline and 3 months after starting treatment for tuberculosis; 443 had persistent euglycemia (HbA1c< 6.5), 118 had persistent hyperglycemia (HbA1c> 6.5), 26 had transient hyperglycemia (final HbA1c level< 6.5), and 1 had incident hyperglycemia. The patients with transient hyperglycemia had worse outcomes, including treatment failures, recurrent tuberculosis, and all-cause mortality. The incidence rate of unfavorable outcomes per 1000-person weeks was 4.76 (% CI: 4.01–5.64) for the entire cohort and was 9.21 (95% CI: 4.96–17.13) for patients with transient hyperglycemia.
