Introduction Pleomorphic adenoma (PA) is one of the most common tumors of the parotid gland and often presents with a very thin pseudocapsule. In addition to lateral (LP) and total parotidectomy (TP), extracapsular dissection (ECD) is also performed as a minimally invasive therapeutic procedure. It has been repeatedly discussed whether the exposed capsule of the PA (" bare area") is responsible for recurrences. The aim of the study was to compare ECD with other surgical techniques regarding to the " bare area" and other capsular structures.

Material and methods As part of a retrospective study (2014-2020), histologic measurement of capsular structures was performed in 132 patients with operated PA; patients with recurrences were excluded.
