Background
Intussusception of the appendix is a relatively rare condition with an estimated incidence of 0.01%. It can mimic several clinical conditions including acute appendicitis or be mistaken as a cecal polypoid lesion on radiological imaging making the diagnosis before or during surgery is very difficult.
Case presentation
A 6-year-old female presented to our unit with complaining of colicky abdominal pain and repeated vomiting for 3 days with no history of constipation. Abdominal examination revealed mild right iliac fossa tenderness and mild abdominal distension. Abdominal ultrasound revealed subhepatic intussusception mass measuring about 3 × 4 cm suggesting underlying Meckel's diverticulum. Three trials of hydrostatic reduction under ultrasound guidance were failed. Abdominal exploration revealed appendiceal intussusception mimicking cecal mass with failed trial of reduction.
