A68-year-old Hispanic male with history of sleep-ing with soft contact-lens presented with a severe corneal ulcer in the left eye. His visual acuity was HM. Examination revealed limbus to limbus corneal infiltration with active melting and a 30% hypopyon (A). Corneal cultures were collected, and topical broadspectrum antibiotic therapy (vancomycin 25 mg/ml and tobramycin 14 mg/ml) were started. After 48hrs the corneal melting continued to progress. Rose-Bengal Photodynamic Antimicrobial Therapy (RB-PDAT) was performed, consisting of 0.1% Rose-Bengal solution applied to the deepithelized cornea for 30 minutes, followed by irradiation with a custom-made 525 nm green LED for 15 minutes (5.4 J/cm2).
