We strongly agree with the supportive, yet cautionary, opinions and questions expressed in the recent Lancet Oncology Editorial about exercise and cancer treatment. 1 With burgeoning clinical and research interest in the adoption of exercise as a standard of care in oncology, it is both timely and appropriate to consider the evidence in the context of pragmatic concerns about delivering exercise services for people with cancer, and the growing desire for exercise services by patients and their health-care teams. Evidence supports that interventions based on cancer exercise guidelines (eg, American College of Sports Medicine [ACSM] and Exercise and Sports Science Australia [ESSA]), most recently endorsed by the Clinical Oncology Society of Australia (COSA), do improve numerous physical and psychosocial outcomes in people with cancer.
