Methods: Geriatric oncology clinic screened new patients≥ 70 years old with Senior Adult Oncology Program 3rd version (SAOP-3). During visit, nursing administered Mini-Cog (MC): three-item recall and clock drawing test. GCOP ordered based on abnormal MC to evaluate adherence. Patient and carer were included to review safety and adherence. Feasibility measures studied were documentation, assessment, implementation, and completion. Results and Conclusion: Total of 1044 patients were screened. Baseline MC completed on 71%(n= 743). Nearly two thirds were females with mean age 77.3 and mean age of males 78.1. Abnormal MC identified in 24%(n= 246). GCOP consult ordered for cognitive impairment in 45%(n= 110). Totals are underestimated due to inconsistent electronic documentation. Time of consult ranged from 45 to 90 minutes.
