Medication errors (MEs) are common worldwide and affect patients, their families, society, health care providers, and the whole health care system. MEs can occur at each stage of the cycle from prescribing, transcription, dispensing to administration. This means that all health care professionals, including doctors, nurses, and pharmacists should be involved in an approach to preventing the MEs. This chapter aims to discuss the MEs -related issues such as history; classifications; causes; detection; reporting; management; as well as highlight the challenges of MEs and their reporting in the developing countries. This chapter also provides recommendations to overcome the challenges in order to improve practices in the developing countries.
