Medicare’s Hospital Readmission Reduction Program financially penalizes hospitals for excess readmissions after specific episodes of care for multiple conditions, including pneumonia. Detailed data about the clinical characteristics of these patients readmitted following a diagnosis of pneumonia are unknown at our institution. Previous quality improvement work done by our institution found significant heterogeneity in the population of patients readmitted after acute myocardial infarction, leading to multiple different interventions to reduce readmissions. Thus, in this present quality improvement study, we aimed to characterize patients readmitted within 30 days of an admission for pneumonia with the goal of identifying strategies to reduce readmissions.
