Evidence suggests that medical errors are most likely to occur during care transitions, i.e., handoffs from unit to unit or discharge home. Lapses in coordination and communication, poorly delineated professional roles and responsibilities, and lack of patient/family engagement are often associated with resulting patient harm. This manuscript describes a small sample interprofessional learning experience executed within the acute care setting, with the goal of positively affecting Interprofessional Education Collaborative (IPEC) competencies, attitudes toward teamwork, and professional role development among nursing and medical students. Nursing and medical students engaged in a 4-week learning activity centered on discharge planning of patients on an inpatient medical-surgical unit.
