Background: Degenerative cervical myelopathy (DCM) is the most common cause of spinal cord dysfunction in adults. Although proposed by many, to date no clear algorithms with the aim of guiding surgical strategy in complex cases still exists. Main objective of our work is to propose and present a new scoring system (Cervical Surgical Score, CSS) to help in identifying the best approach for complex cases of DCM.

Methods: All patients operated on for DCM at our Institution between December 2021 and December 2022 were prospectively enrolled. During their Spine Surgery outpatient clinic, a specific surgical indication was offered and for all cases clinical and radiological data were collected.
