To the Editor: As medical director of an inpatient substance use treatment facility operated by University of North Carolina (UNC) Health, I supervise psychiatry residents who are completing addiction medicine rotations. Through this work, I have come to appreciate the amount of confusion that exists among trainees as well as faculty surrounding medication treatment for opioid use disorder (OUD), and much of the confusion over opioid addiction treatment relates to its regulation. Treatment of OUD is a heavily regulated area of medicine, and this high degree of legal control has contributed to a perception that treatment of addiction is fundamentally different from the treatment of other medical illnesses, and engendered a sense of unease within many providers that has resulted in an unwillingness to engage in the practice of offering medication treatment for OUD.
