Methods: From December 2022 to August 2023 we encountered 12 ESRD patients with sub-optimal (< 4 mm and> 3 mm) basilic veins, and inserted 5 mm or 6 mm BCAGs augmented with a 7.5 cm or 10 cm x 8 mm covered stent. Basilic veins were accessed using the Seldinger method. Ultrasonographic vein mapping was done prior to the procedure. Results: Four right-arm and eight left-arm grafts were inserted in 12 elderly patients (average age 70 years, range 48e84), of whom 11 were female, and 1 male. Two patients had 6 mm BCAGs, and 10 had 5 mm BCAGs. Three patients had 7.5 cmx 8 mm covered stents, while nine had 10 cm x 8 mm. Average operation time was 68.5 minutes. One patient passed away two months after graft insertion, and one patient developed steal syndrome. Primary and primary assisted patency was 83% and 50% at 3 and 6 months, respectively.
