The medication use process includes the following stages: prescribing, dispensing, administering, and monitoring of medications. The prescribing stage is a key target for improving medication-related safety, as prescribing errors are a major source of medication-related problems, such as adverse drug events. 1 Older people are particularly vulnerable to prescribing errors such as inappropriate prescribing, polypharmacy, and adverse drug events. 2 Older people tend to have multiple conditions, with 65% having 2 or more chronic conditions such as diabetes and heart failure, 3 requiring multiple drug treatments.
