The leptin-melanocortin pathway (LMP) is an essential component of hypothalamic energy balance regulation. Heterozygous variants in genes encoding for components of the LMP have been associated with obesity [1, 2]. Bariatric surgery is the most effective therapy for weight loss and improvement of obesity-related comorbidities. In the USA, laparoscopic sleeve gastrectomy (SG) is the most frequently performed bariatric surgery (61.4%)[3]. We recently showed that approximately 10% of the population undergoing Roux-en-Y gastric bypass (RYGB) are carriers of a genetic variant in the LMP, and that carriers who undergo RYGB or transoral outlet reduction (TORe) after RYGB have poorer weight loss outcomes and a higher rate of weight recurrence than non-carriers [1, 4].
