To the Editor: The recent report by Hong et al. 1 describes a 66-year-old man with
spontaneous intracranial hypotension (SIH) presenting with an 18-month progressive history
of cognitive impairment and postural headaches. MRI revealed characteristic diffuse dural
enhancement and a “sagging” brain with mid-brain effacement, incisural block, pontine
