The use of RapidArc volumetric-modulated arc therapy to deliver stereotactic radiosurgery and stereotactic body radiotherapy to intracranial and extracranial targets

Twenty-three targets in 16 patients treated with stereotactic radiosurgery (SRS) or
stereotactic body radiotherapy (SBRT) were analyzed in terms of dosimetric homogeneity,
target conformity, organ-at-risk (OAR) sparing, monitor unit (MU) usage, and beam-on time
per fraction using RapidArc volumetric-modulated arc therapy (VMAT) vs. multifield sliding-
window intensity-modulated radiation therapy (IMRT). Patients underwent computed
tomography simulation with site-specific immobilization. Magnetic resonance imaging fusion …

Results All RapidArc VMAT plans were delivered in a single arc, while IMRT plans required
7–14 fields (median: 9) for delivery. Target volumes ranged from 0.5–181.9 cm 3 (median,
11.7 cm 3). The VMAT fractions were delivered more rapidly than IMRT fractions, with
mean±SD beam-on times of 2.9±1.4 minutes (range, 1.3–7.4 minutes) vs. 12.5±7.1 minutes
(range, 5.1–30.8 minutes), respectively. The median MU for VMAT and IMRT fractions were
1,634 (range, 925–7,265) and 2,536 (range, 985–8,253), respectively. For the VMAT plans …
