200 cases of cerebral palsy with hip deformities requiring operation have been reviewed. Of these, 104 had radiographs of the hip, and in 63 dislocation of the hip was found. These hips were all flexed and adducted, and showed a normal acetabulum. In 22 cases a series of radiographs were available showing that the dislocation had developed in a hip which had once been radiographically normal. In 24 cases operation had been carried out to stabilise the hip.

Surgical procedures at the hip may thus be necessary in cerebral palsy at 3 stages: (I) At an early stage to prevent progressive development of hip deformity. (2) Where fixed hip deformity has become established and surgery may be the only means of releasing contraction and restoring muscle balance so that education in standing and walking can be instituted. (3) In those cases of long‐standing hip deformity in association with dislocation where surgical stabilisation of the dislocated joint is an essential part of rehabilitation.

Les déformations des hanches dans la paralysie cérébrale et leur traitement

200 cas de paralysie cérébrale avec déeformation des hanches, nécessitant une opération, ont étéétudiés. 104 Centre eux avaient des radios des hanches et parmi ceux‐láa, on a trouvé 63 dislocations de la hanche. Ces hanches étaient en flexion et adduction, mais l'acetabulum était normal. Dans 22 cas, on disposait de radiographies en série qui montraient que la dislocation s'était développee sur une hanche radiologiquement normale. Dans 24 cas, on a procédéà des interventions pour fixer la hanche.

L'intervention chirurgicale sur la hanche est indiquée dans la paralysie cérébrale dans trois stades: 1°)‐au deé Abut, pour prévenir un développement progressif et la déformation de la hanche; 2°)‐lorsque la déformation de la hanche est fixée et que la chirurgie est le seul moyen possible dé réduire la contraction et de rtéablir un équilibre musculaire de faqon à ce qu'on puisse réé duquer la station debout et la marche; 3°)‐dans les cas de déformation de la hanche ancienne, associée à des dislocations, oúu I'intervention sur l'articulation disloqutée est un élément essentiel de la réhabilitation.

Deformationen der Hüften bei Zerebrallähmung und ihre Behandlung

200 Fälle von Zerebrallähmung mit Hüftendeformationen, die eine Operation erfordern, sind untersucht worden. 104 unter ihren hatten Radiographier—der Hüufte und bei 63 wurde eine Verrenkung der Hüfte gefunden. Diese Hüften waren alle gebeugt und adduziert und zeigten ein normales Acetabulum. In 22 Fällen verfügte man über Reihen‐Radiographier die zeigten, dass sich die Verrenkung in einer radiologisch normalen Hüfte entwickelt hatte. In 24 Fällen wurde eine Operation ausgefährt, um die Hüfte zu befestigen.

Chirurgische Eingriffe auf de Hüfte können also bei Zerebrallähmung in 3 Stadien notwendig sein: 1° Am Anfang, um einer progressiven Entwicklung der Deformation der Hüfte vorzubeugen. 2° Wenn die Deformation der Hüfte fixiert ist und Chirurgie das einzige Mike1 ist, um die Kontraktion zu beseitigen und das Gleichgewicht der Muskeln wiederherzustellen so dass eine Übungstherapie für das Stehen und das Gehen durch‐geführt werden kann. 3 ° In den Fällen von lange bestehender Hüftendformation, mit Verrenkung, wo eine chirurgische Befestigung des verrenkten Gelenks ein wesentlicher Teil der Rehabilitation ist.
