The incidence of aesthetic and functional complications of rhinoplasty can be decreased by
preserving supportstructures of the nose. This is particularly important in patients at high risk
for collapse of the middle nasal vault (cartilaginous nasal vault). Aggressive surgical
manipulation of the upper lateral cartilages can result in excessive narrowing of the middle
